If you think that you have been a victim of medical malpractice, you need to go see an attorney as soon as possible. Full Medical History

You will need to give your attorney your full medical history, both prior to whatever malpractice incident that you are suing for and after the incident. The reason that you want to do this is that you want to show the reason you sought that particular treatment as well as how that incident has affected your life. You will need to get your records from every doctor that you see so that your attorney will have the best information about what your life is like. Don't worry about being embarrassed by anything; your attorney is there to help you, not judge you.

Signed Releases

You won't necessarily give your attorney the signed releases that you have left with your doctors, but you can tell them that you have signed the releases. The releases will allow your doctor to talk to your attorney about your case. Without that release, your doctor can't even acknowledge whether or not you are their patient. Your attorney will be able to talk to them all they want, but the doctor can't give them answers. But with the release, that's not an issue. One nice thing about the release is that you can limit what information the doctor can give the attorney. You can also limit how long the release will last. The default time on releases is one year from the date that it is signed, but you can limit that to six months or expand the time limit if you think that your case may go on past that.
